Basic Needs

The Basic Needs Specialty Clinic helps address the social factors that impact health by connecting patients with food, essential supplies, community resources, and supportive services. Our goal is to improve health beyond the exam room by ensuring patients have access to the resources they need to thrive.

Specialty Clinic services include
-
Grocery Delivery Program: Healthy groceries delivered to eligible patients experiencing food insecurity.

Care Package Distribution: Food, hygiene products, seasonal essentials, and other basic necessities provided to patients during clinic and through community outreach to the unhoused population of Sacramento.

Hygiene & Essential Items: Distribution of hygiene kits, first aid supplies, socks, sunscreen, hand warmers, and other necessities for patients and individuals experiencing homelessness.

CalFresh Enrollment Assistance: Help determining eligibility and completing CalFresh applications.

Community Resource Navigation: Connecting patients with housing, transportation, clothing, food, healthcare, legal, and mental health resources.

Community Outreach: Providing resource guides, educational materials, and support at health fairs, schools, and community events throughout Sacramento.

Hypertension Clinic

Hypertension Clinic provides accessible, patient-centered care focused on preventing and managing high blood pressure through education, screening, and individualized treatment plans.

Specialty clinic services include:

Hypertension Management: 1-on-1 consultations with cardiologist Dr. Lopez to develop personalized blood pressure management plans.
-
Free Blood Pressure Screenings and Assessments: Routine screenings, risk-factor evaluations, and monitoring for patients with hypertension or at risk for developing hypertension.
-
Hypertension Reduction Program: A specialized program focused on lifestyle modifications, nutrition counseling, exercise recommendations, and patient education to help decrease and control high blood pressure.

Referrals: Connections to community resources, specialty care providers, nutrition services, cardiovascular programs, and long-term healthcare support as needed.
